4(a)        State five causes of accident in the store

  (b)      Explain the following documents used in the store:

  1. stores requisition;
  2. invoice;
  3. issue note;
  4. goods received note;

Majority of the candidates attempted question 4 (a) part correctly while in the 4(b) parts of the question candidate wrongly explained that invoice is prepared by the storekeeper, issue note prepared by the supplier and could not differentiate between goods received note and invoice.

The responses expected from candidates are:

4(a)      Causes of accidents in the store:

  • Improper material handling by the storekeeper;
  • Faulty equipment that breaks down always at a vital movement also causes accident in the store;  
  • Poor storage conditions in the form of bad floor, unstable racks, shelves, bins and general fittings are responsible for accident in the store;
  • Wrong usage of storage and handling equipment;
  • Overloading of storage and handling equipment against manufacturers specifications may result to accident in the store;
  • Lack of supervision of store handling operations by the storekeeper.
  • Untidiness of the store environment and carelessness by store personnel can cause accident in the store;
  • Inadequate safety signs and warnings in the store;
  • Insufficient knowledge in the use of the store equipment;
  • Poor maintenance or servicing of store equipment;
  • Failure to use the protective wears properly;
  • Non-compliance with safety rules and regulations by staff.

 

(b)     Documents used in the Store

  1. Stores requisition: This is a document used to request for material from the store. It contains an authorization to get items in the store prepared by a user department containing quantities and types of items.                                       
  2. Invoice: An invoice is a document sent by a supplier to show details of the materials that have been supplied and the unit and total amount to be paid for the supplied materials.                                                                    
  3. Issue note: Issue note is a document that is prepared by storekeeper to accompany the materials that have been released to user department. It contains the details of the materials that have been issued to users.                                      
  1. Goods received note: This document is used when materials have been delivered or supplied to the store, the storekeeper will prepare a goods received note after the goods have been inspected and accepted into the store.                  
  2. Rejection note: It is a document that indicates the number of goods that have been rejected by the storekeeper for one reason or the other like incomplete quantities, incorrect materials and damaged materials supplied.
